Registration Form Registration Form Please download, complete and mail or fax in the registration form with your check for each child attending Boot Camp. Please select the correct form for the dates you wish to attend. Complete program details are on the forms. Classes will meet Tuesday, Wednesday and Thursday 9:00 a.m. to 12:00 noon. Parents and family are invited to the graduation ceremonies held at 12:15 p.m. on Thursday. Registration deadlines are approximately 10-14 days prior to the start of each term.
